ATTENTION VENDORS: Please include the product manufacturer, product ID, unit price and total price. On the last page include the Total number of items bid" and the "Total amount of bid". Samples may be required prior to awarding of line items. Submission of samples shall be required to be delivered to the appropriate department, during their delivery times, within a reasonable period of time after you are notified to do so. Samples will be at the expense of the vendor. No samples shall be delivered prior to the bid opening. FAILURE TO SUBMIT SAMPLES, WHEN REQUESTED, COULD BE REASON FOR REJECTION. Quantities are estimates, they may be increased or decreased in any amount. Please refer to the Boiler Plate of this specification for more detailed explanation on quantities. Departments have requested that the item numbers identified in the bid, be included on the invoice, so that they can check each delivery and insure proper billing. Part A will be awarded as a line item bid. Part B,C & D will be awarded to one vendor.  Delivery time is 6:00 am to 1:00 pm.

Line items 28 through 36 will be awarded as a "lump sum bid" to one vendor. Vendor must bid all items.

 Line items 37 through 40 will be awarded as a "lump sum bid" to one vendor. Vendor must bid on all line items.
